Benefits from National Health Insurance

Medical and dental checkups

We organize the following health checkup program for individuals who are members of the NHI in Tokushima City. Please take advantage of this program for your health.

How to
apply

During the application period (from April 8 to April 30*), please come to Counter No.7, Health Insurance & Pension Division (Hoken Nenkin-ka) on the 1st floor of City Hall, with your Eligibility Notification.
The period of taking this checkup:
July 1st to December 20th
Please carry the eligibility proof etc. to counter 7, Health Insurance and Pension Division (Tokushima City Hall, 1st floor) during the application period.
(1st term: 8th Apr.~30th Apr.,
2nd term: 8th Sept. ~30th Sept.)
Examination period:
1st term: 1st Jun. ~ 31st Oct.
2nd term: 1st Nov. ~ 31st Mar.

Make a reservation by phone or online etc. for the dentist of your choice. (Choose from the members of Tokushima City Dentists’ Association.)
Please carry either your NHI card, My Number Card or your eligibility proof. Appointment is available from June 1 until August 31st

Treatments that NHI does NOT cover

NHI does not cover the medical treatments specified below, and you are required to bear all expenses for them.

  • Treatments and medication that health insurance programs do not cover
  • Additional expenses for hospitalization in a room with fewer beds
  • Personal medical checkup
  • Preventive injection
  • Cosmetic surgery
  • Orthodontia
  • Injuries and diseases that workmen’s accident compensation insurance can cover
  • Injuries and diseases caused by criminal acts or fighting

Report of injuries and diseases caused by a third party

As a rule, medical expenses for the treatment of injuries and diseases caused by a third party such as a traffic accident should be paid by the guilty party. You will be required to submit a report to us when you receive treatment from a doctor. If you are involved in a traffic accident, report it to the police and ask them to give you a Traffic Accident Certificate. Then come to our office at the Insurance & Pension Division (Hoken Nenkin-ka.)

Required materials:

  1. Eligibility Certificate
  2. ID of the person completing the application (e.g. Residence card, Special permanent resident certificate, Driver’s license, etc.)
  3. Traffic Accident Certificate issued by the police
  4. Seal (inkan) (excluding rubber and self-inking stamps)

If you undergo expensive medical treatment

If you need to receive expensive medical treatment, either as an inpatient or outpatient, you can reduce your-out-of-pocket expenses by presenting a Maximum Copayment Certificate (Gendogaku-Nintei-sho) at the medical facility. To obtain this certificate, please apply in advance. However, if you are 70 years of age or older and your income category falls under “Active Income Level 3” or “General,” you don’t need to apply for the certificate, as it will be issued automatically.

Requirements:

  1. Eligibility Certificate
  2. The Individual Number (My Number) card or notification of the head of household and the person receiving the treatment
  3. ID of the person completing the application (e.g. Residence card, Special permanent resident certificate, Driver’s license, etc.)

* If you provide the My Number Insurance Card, payment over the coverage limit (under the high-cost medical expense benefit system) will be exempted without prior procedure.
In addition, it is not necessary to apply for the “Eligibility Certificate for Ceiling-Amount Application (Gendogaku Tekiyo Nintei-sho)” or “Eligibility Certificate for Ceiling-Amount Application and Reduction of the Standard Amount of Patient Liability (Gendogaku Tekiyo/Hyojun Futangaku Gengaku Nintei-sho)” if you have My Number Insurance Card. We recommend that you obtain and utilize your My Number Insurance Card.
Note: Those who have arrears of National Health Insurance fees or have failed to declare income may not be eligible for this medical expense benefit nor the correct coverage limit amount may not be applied.

Funeral allowance

When those who are insured by the National health insurance of Tokushima City pass away, a funeral allowance of 20,000 yen will be provided to the person who conducts the funeral (moshu).

Required materials for the application:

  1. Seal (inkan) (excluding rubber and self-inking stamps)
  2. Bank account number (of the person conducting the funeral)
  3. Document that proves the applicant conducted the funeral
  4. ID of the person completing the application (e.g. Residence card, special permanent resident certificate, Driver’s license, etc.)
  5. Eligibility Certificate
