Application processes
Your Residence Card or Permanent Resident Card and your Individual Number (My Number) notification card or Individual Number (My Number) card are required to complete an application along with the following items:

*1: If you don't have "My Number Card" (linked with your health insurance qualification), please bring "Eligibility Certificate."
Note: All reports should be submitted within 14 days of the incident. If you fail to report in time, you are required to pay the insurance premium retroactively from the month in which your qualification and obligation began.
- e.g.If you leave a company in June and apply to join NHI in September, you will have to pay the premium for June through September retroactively.
(The retroactive levy goes back 2 years at the maximum.)
