Application for Change of Childcare Need Classification/Notification of Changes to Facility Use Application
If part of the information you wrote in your application changes after you begin enrollment (address change, family/work situation, etc.) you must complete an “Application for Change of Childcare Need Classification/Notification of Changes to Facility Use Application(PDF形式:119KB).” (Referred to as the "Application of Change.")
The change concerning your childcare need determination will happen on the 1st of the month after the date of whichever of the following occurs later:
① the date the change occurs
② the date you submit the notification of change.
However, for some changes (marriage/job searching/etc.) the childcare need determination change will traditionally happen the month after the change officially happened (or the same month if the event happened on the 1st of the month).

Note 1: In the case you move out of Tokushima City, the day you must withdraw from your facility will be the last day of the month you will move (or the last day of the month before you moved if you moved on the 1st of a month.)
- If you wish to continue using the same childcare facility your child was enrolled at before moving, you must complete the proper procedures at your new municipality. Please contact the Childcare Division in Tokushima City for more information. (Please understand you may not be able to continue enrollment at your facility.)
Note 2: No matter when your (pregnancy) leave began, if the first day you wish to use a childcare facility falls within the period of 2 months before to 2 months after your expected due date month, you will fall under the “pregnancy/post birth” childcare need requirement.
In the case you began utilizing a childcare facility under the pregnancy/post birth
requirement, your child will have to leave their facility as soon as the validity period is over. (You must complete a new application if you wish to continue enrollment)
- Please note that in the case your spouse takes childcare leave at any point during your pregnancy/post birth childcare need validity period, you will lose your childcare need determination. (Your child with be withdrawn from their facility)
Note 3: If you plan to take childcare leave (even if you do not necessarily need childcare support during this period) and you have a child currently enrolled at a childcare facility, in the following situations, even after your period of a validity for pregnancy/post birth ends your child can continue their enrollment at the same facility:
1. Your older child will be entering elementary school the next school year, so it is determined to be best if they continue enrollment at their current facility in order to not (suddenly) change their developmental environment.
(The currently enrolled child MUST be part of the 5th year class at the time the guardian’s childcare need requirement is changed (= the end of the month that one’s pregnancy/post birth period of validity expires))
2. It is determined that suddenly changing the current (educational/childcare)environment of the older child would be detrimental to the child’s development environment:
(In the situations where: the childcare leave period ends by/before day before the newborn child’s 1st birthday and your older child has been enrolled at their current facility for over 3 months before the birth of the newborn child)
Requests Concerning Certificates Without Official Stamps
As of July 2021 it is no longer necessary to receive official stamps on the “Necessary Documents to Prove Childcare Need” listed above.
While it is now not necessary to receive a stamp from your employer on your employment certificate (etc.), we still ask that you provide one of the following when submitting your Employment Certificate, Certificate of Illness/Disability, etc., in order to help prove your situation.
Employment Certificates Without an Employer’s Stamp (etc.)
- A document (email, FAX, etc.) showing how the guardian received the employment certificate (such as a screenshot/copy of an email with the attached certificate, FAX form, envelope the certificate came in, etc.)
Certificates of Illness/Disability, caregiving situation forms (etc.) without a stamp from a medical institution
- A document proving that the guardian received the certificate from a medical institution such as a document showing the relative has been going to a clinic, envelope received from clinic with the certificate, etc
Notes
In the case you have none of the additional documents above, in order to confirm your degree of childcare need, the Childcare Division may directly contact your employer/medical institution (etc.).
WARNING! (Illegal Alteration of Documents)
Altering information (creating new/changing information) on your company’s written
or digital version of your employment certificate, whether or not the document contains the stamp of your workplace or not, is considered counterfeit of private documents/illegal alteration of private documents/unauthorized creation of electro magnetic records In the case it is discovered that you preformed any of these illegal actions, you will have your enrollment decision into a childcare facility revoked or your child will be removed from their facility.
When Your Period of Validity Ends
Those who wish to continue/begin using a childcare facility after their period of validity (written on their Childcare Need Classification Certificate) ends mustmust submit any necessary forms by the 15th of the month their period of validity ends. (Please consult with the Childcare Division directly to confirm which forms are necessary for you.)
(You do not need to complete an application (etc.) if your child will change from a Type 3 to a Type 2 Childcare Need determination (i.e. your child turned 4yrs old under a Type 3 determination). This change will happen automatically.)
If You Lose/Misplace Your Childcare Need Classification Certificate
Please submit the same Childcare Need Determination Application & Facility Use Application Form(PDF形式:211KB) if you lose your certificate.
(Please be sure to keep your certificate in a safe place as to not lose it.)
