Lump-Sum Birth Allowance

Lump-sum birth allowance (up to ￥500,000 per child) will be provided for those who are enrolled in a national or employees’ health insurance program. For more information, please contact your insurance company.
The national health insurance program of Tokushima City will provide ￥500,000 per child as its lump-sum birth allowance. The same benefits will be paid in the case of stillbirth or miscarriage if the woman was pregnant for 12 weeks or longer (a medical certificate signed by your doctor is required).

• Eligibility Certificate
• “My Number Card” or notification letter of the head of a family and the woman who gave birth
• ID of the person applying for the eligible applicant (e.g. Residence card, special permanent resident certificate, driver’s license, etc.)
• Document showing your bank account number (The bank account must be under the name of the individual who gave birth or the head of the household)
• Maternal & child health handbook (Boshi Kenko Techo)
• Receipt
• Document showing your refusal of direct payment (issued by the hospital)

If you intend to use this allowance to pay for the medical cost of your delivery, we can send your allowance within the limit of the lump-sum birth allowance directly to the hospital. Please contact the hospital if this is what you want to do.

• If your childbirth expense is within the limit of the allowance, you may receive the remaining balance at a later date.
• If you decline to send your allowance directly to the hospital, or if you give birth abroad, your allowance will be sent to you or the head of your family upon request.
